Profile (Total Cholesterol, HDL, LDL, Triglycerides), SGOT, SGPT.

Radiology

Hospital accommodation:
Admissions will be covered for three (3) days per quarter which include free nursing care for the 3 days covered by the membership, these can be carried forward to the next quarter if not used in that quarter upon advance payment for at least two quarters or more.

Emergency & Surgery

• 10% Discount On ICU (except injectable and pharmacy)
  Note: Surgical procedures are NOT covered by membership, however the total
  admission without pharmacy and injectable bill will be given a discount of 5%.
• Use of operating theatre
  Please note that the use of oxygen, anesthesia gasses are charged and will appear on your bill
• Ambulance charges will be given a 15% discount within 20km radius of Forest Park Specialised Hospital.

Medications

• 10% discount on all medicines

Exclusions Forest Park specialized Hospital Membership will not cover the following:

• Reports examinations and test required for immigration, visas, insurance policies, employment, and admission to schools or Universities, court medical reports, fitness examinations, adoption of children and reports and tests for retirement because of ill health.
• Annual health checkup (Full medical examines).
• Operations and procedures.
• Procedures for orthodontic, periodontal and elective maxillofacial and oral surgery.
• Conditions for which costs are recoverable from another party.
• Conditions arising from self-inflicted injuries, suicide or attempt to commit suicide whether or not the person was criminally accountable
• Injuries arising from professional sport and power driven vehicle sport.
• Medication use to prevent such injuries.
• Injuries arising from actions on account of criminal transgression on which the member were found guilty. This includes: participation in illegal strikes.
• Examinations, test and treatment of impotence, infertility or artificial insemination or IVF.

Terms and Conditions

• All chronic cases will be given a discount of 5% under outpatient visits after the subscription on the membership before its activation.
• Annual payments will be offered 5% discount for both membership classes.
• Group members 5 or more will get 15% discount.
• Membership will only be activated after 30 days of payment.
• Pap-smear can only be taken twice a year (i.e. once every six months).

Minimum deposit: K12,500 for a family of three (3)

Forest Park specialized Hospital membership will not cover the following:

• Hospital admission for any Chronic or Pre-existing conditions and Pregnancy related admissions are not covered for the first two years.
• Subscription renewals not paid within thirty days of due date will result in termination of Membership. Once Membership has been terminated, the member will be required to join as new member.

All Procedures will be subject to a 25% discount that will apply upon activation of the membership.

Cancelation of Fund

• 30 days or more notice will not attract penalties
• 14 days to 29 days’ notice will attract a penalty of 25%.

• 7 day to 13 day notice will attract a penalty of 50%. 

• 6 days or less notice will attract a 75%penalty.
